I'm not familiar with McAfee, so I'm not entirely sure any application-specific problems that would cause this. However, it does sound like you may need a repair installation of McAfee.
Also, the scanner will only quarantine files that it cannot clean. For example, I once had an infected file that couldn't be healed because it was in a ZIP archive and needed to be uncompressed before healing could take place. I'm sure there are other situations that would have the same result.

Best thing you could ever do for yourself is use the McAfee removal tool and install AVG or Avira AntiVir PE.
You'll end up with a lot less bloat, a faster machine and far less problems.
By Any chance do you have Internet Explorer Beta 8 Installed? if so, uninstall the beta and return to Internet Explorer 7.
McAfee and IE 8 (Internet Explorer) Clash. So if you want to stay with your current Anti-Virus Provider then Re-install Internet Explorer 7.
